Before you are permitted to enter Lithuania, you will need to fill out a Lithuania Registration Form. This is needed by all overseas residents of Lithuania, though they may not yet have a resident visa. The Ministry of Foreign Affairs in Lithuania will allow you to obtain a visa so as to go to Lithuania, however you will still need to complete the registration form before you may travel. You can still use your passport as a valid form of identification

You also have to indicate whether you want a health insurance card. You will be required to give details such as name, contact information, telephone number, contact number, date of birth, passport expires, and payment methods. You will also have to list any other countries which you travel to in your enrollment form, such as the U.S., U.K., France, Germany, and Italy. A negative diagnosis in your wellbeing authorities' records may prevent you from obtaining a health travel insurance policy in Lithuania.

Your registration form may ask you to record all of your vaccinations, such as your first and next set of immunizations. If you've received any other vaccinations as your very first trip to the vaccination center in Lithuanian capital Vilnius, then you'll have to list these as well. Your enrollment form will specify if your vaccinations need to be revived, and you will have to indicate whether you wish to continue your remedies at the centers. For those that did not receive a vaccination certificate at the time of your first visit to the law centre in Vilnius, you will still need to indicate these in your enrollment form.

List all your personal products. These include: passport, vaccination card, airline tickets, airline tickets, and any prescriptions. You will also be required to list all of your personal items that are in your luggage or in your car. It's critical to indicate if any of your prescription medications have been in your possession, such as: aspirin, vitamins, and blood pressure drugs.

List all of your personal illnesses and ailments. You'll need to clarify if your disease or condition isn't yet diagnosed. If it is not a contagious disease that can spread easily, you will also need to explain if it has a specific name (i.e. strep throat). The sole infectious ailments that require an entry to the vaccination centre's database are the ones which are mechanically contracted through contact, inhalation, or blood.

Organize your marriage/divorce details. The only entry necessary for this component of your Lithuania Registration Form is the married name. It is also advisable to indicate whether you had been previously divorced or currently being married. The health authorities will also need to know the names of any children or individuals who are living with you. Be sure to list the legal titles of these individuals too, including the city and city they reside in. Each these details should be on your registration form for Lithuania.
